First Appointment—What to Bring
We recommend that you arrive 10-15 minutes prior to your scheduled appointments to ensure that you have enough time to complete any necessary paperwork. In order for your visit with us to run smoothly and efficiently, we recommend that you bring the following items with you on your first visit.
Photo ID and Insurance Cards
Effective June 1, 2016, WOSM will no longer be submitting office visits to insurance companies on the patient’s’ behalf. We will provide our patients with all the necessary paperwork and a user-friendly coded cover sheet along with instructions for submission. By empowering our patients to interact directly with their insurance carriers, there will be less confusion regarding service coverage and reimbursements will go directly to the patient rather than to WOSM (necessitating a refund to the patient with its incipient delays and confusion). We will continue to submit surgery bills on the patient’s behalf and our billing and collection department is always available for questions or problems. We appreciate your understanding as we are forced to adjust our practice to the ever changing health insurance environment. We apologize for any inconvenience this may cause. We do ask that you bring your photo ID and insurance card to maintain in our PM system for those cases where the physicians will direct bill for procedures and surgical cases.
Medical Records
If this is a recurrent or chronic problem, please bring the following:
- Pertinent medical records
- Imaging studies (including x-rays, MRIs, or CT scans)
- Radiology reports
- Prior operative reports,
- Any other information you feel our physicians may find helpful in their assessment of your condition.
Medication List
Please bring a list of all medications that you are currently taking, along with dosages and frequencies. Please include supplements and over-the-counter medicines that you take on a regular basis.
Comfortable Clothing
Please wear clothes that allow you to show the affected body part easily. For women with an upper extremity injury, please bring or wear a sports bra or tank top without underwires (these will not interfere with x-rays). Those with a lower extremity injury should wear or bring shorts.
